Position Title: Home Health Social Worker (HHSW)
Description: The HHSW works in collaboration with the physician and other members of the home health professional team in identifying social problems, their severity, and their interrelatedness to the medical situation. The HHSW participates in the development of the treatment plan by preparing social histories according to specific guidelines to augment existing service or as a guide in determining or changing level of service. HHSW assesses the patient at each visit and will refer to community services for assistance with care based on the patient’s prognosis and medical needs or possible financial difficulties.
Qualifications: Bachelors of Social Work and two (2) years of relevant work experience for a home health agency, hospice agency, or hospital preferred. Bilingual encouraged to apply.
Position Title: Foster/Adoptive Home Assessor (FAHA)
Description: The FAHA works in collaboration with the courts, attorneys, DFPS (CPS), child placing agencies, and other related professionals in regards to the fostering/adoption of children. The FAHA participates in the development of various reports which may include the health, genetic, emotional, social, marital, or mental health history of a child or adult as outlined by the particular entity requesting the evaluation and/or report. The FAHA will be responsible for determining whether a home environment is safe, the stability of an individual/couple in providing care for a child who is not biologically related to them, and in completing related reports for the court.
Qualifications: Bachelor's Degree in Social Work or related human service field and two (2) years of relevant work experience for a child placing agency preferred.
Position Title: Behavioral Health Therapist (BHT)
Description: The BHT will provide psychosocial evaluations, psychotherapy, and counseling for Texas Medicaid Clients, both children and adults, who have intellectual or developmental disabilities and are experiencing a significant behavioral health issue that is causing distress, dysfunction, or maladaptive functioning. The BHT works in collaboration with local agencies enrolled in the Texas Department of Aging and Disability Services (DADS) HCS and TxHML programs to supplement the services received by providing separate, psychotherapy and counseling. The BHT will create an individualized treatment plan, identify and establish goals, teach skills, and provide therapy services that will assist the client in reaching those goals. Services are provided in homes or community based offices.
Qualifications: Must hold license as either a Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Counselor (excluding temporary licenses or licenses which are due to expire within 30 days). Experience working with children, families and adults is encouraged. Bilingual encouraged to apply.
Position Title: Mental Health Counselor (MHC)
Description: The MHC will provide psychosocial assessments, individual and family counseling, and group counseling to clients, both children and adults, who are receiving services from the Department of Family and Protective Services. The MHC will create an individualized treatment plan, identify and establish goals, teach skills, and provide therapy services that will assist the client in reaching those goals. Services are provided in homes or community based offices. MHC may also be requested to appear for court testimony, deposition, or mediation if subpoenaed by the court and must appear as a condition of employment.
Qualifications: Must hold license as either a Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Counselor, or Psychologist (excluding temporary licenses or licenses which are due to expire within 30 days). Experience working with children, families and adults is encouraged. Bilingual encouraged to apply.
Position Title: Geriatric Social Worker (GSW)
Description: The GSW will provide psychosocial evaluations, psychotherapy, and counseling for Medicare clients and their family who are experiencing a significant behavioral health issue that is causing distress, dysfunction, or maladaptive functioning. The GSW may work in collaboration with physicians, nurses, clinics and/or home health agencies in identifying social problems, their severity, and their interrelatedness to the medical situation. The GSW will create an individualized treatment plan, identify and establish goals, teach skills, and provide therapy services that will assist the client in reaching those goals. Services are provided in client’s home.
Qualifications: Must hold license as a Licensed Clinical Social Worker (excluding temporary licenses or licenses which are due to expire within 30 days). Experience working with geriatric clients or adults with chronic illness is encouraged. Bilingual encouraged to apply.
Position Title: : PRN Foster / Adoption Home Assessor (“FAHA”)
The FAHA works in collaboration with local courts, attorneys, DFPS (CPS), child placing agencies, and other related professionals in regards to the fostering / adoption or general care of children. The FAHA participates in the development of various reports, such as: Kinship Assessments, Foster & Adoption (FAD) Assessments, Social Studies, private Pre- and Post-Placement Reports, Interstate Compact on the Placement of Children (ICPC) and Health, Social, Emotional, Genetic History (HSEGH) Reports. During these assessments, the FAHA will be expected to interview individuals in a professional and personable manner regarding their health, genetic factors, emotional stability, social supports, marital status, and/or mental health history of a child or adult as outlined by the particular entity requesting the evaluation and/or report. The FAHA will be responsible for determining whether a home environment is safe, the stability of an individual or couple in providing care for a child who may not be biologically related to them, and in completing related reports for the court and/or requesting lawyer or agency.
Qualifications: Bachelor's Degree in Social Work or related human service field and two (2) years of relevant work experience for a child placing agency (mandatory). Must have reliable transportation for traveling to clients’ homes, and would prefer evening and weekend availability since all household members must be present for home assessment.
